New Student Registration for Additional Students
NOTE: A New Student Registration form must be submitted for each student in your family. Once you have successfully submitted one New Student Registration, you will have the opportunity to begin another from the "Confirmation" page.

Translation services are available to assist families registering in the district.
If you need assistance with the registration process you are welcome to come to the District Services Center during Registration Open Office Hours as follows:
During the Summer Months (June, July, August) on Wednesdays from 8:00am- 3:00pm
Regular Open Office Hours will resume in September through May on Tuesdays 1:00pm-3:00pm and Thursdays 9:00am-12:00pm. It is helpful to bring along the Required Documents you have available.

Required Documents: The following documents are required by law for admission into Quakertown Community School District. Acceptable documentation for each item is listed below.
1. Proof of Child's age
Original Birth Certificate
Notarized copy of birth certificate
Baptismal Certificate
Valid Passport
2. Immunizations
3. Proof of Residency
One of the following mandatory documents:
Proof of home ownership:
Current deed,
Property tax bill
Mortgage statement
Agreement of Sale or settlement paperwork
(if just purchasing a home in QCSD)
OR
Current rental lease - please include ALL pages of the lease agreement
PLUS TWO of the following additional documents:
(Note: All documentation must be current, within the last 30 days, and in the name of the parent/guardian registering the child. Photos of Envelopes are not acceptable. See example below.)
Current utility bill
Current credit card statement
Current bank statement
Current paycheck stub
Public assistance or social security
Current homeowner's or renter's insurance
4. Photo Identification of parent/guardian registering the child

ADDITIONAL FORMS THAT MAY BE REQUIRED
Multiple Occupancy: an application for Multiple Occupancy may be required in cases where you and your child are residing with another family within the Quakertown Community School District and you cannot provide the mandatory proof of residence documentation. Please click here for more information regarding Multiple Occupancy.
Guardianship Questionnaire: You must be the legal parent/guardian of the child you are registering. If you are not the legal parent, you must provide legal documentation to show that you are the child's guardian. If the student is living in a household of a school district resident who is not the parent or legal guardian but will assume responsibility for the student, the resident must complete this questionnaire. Registration will not be considered until the District has reviewed and approved this questionnaire. Upon review/approval, the District will provide a 1302 Affidavit to the undersigned resident and parent. The affidavit will need to be completed, signed, and notarized prior to registration.
